  Said founder had no prior medical experience and said procedure has since been linked to a lawsuit alleging a patient death following a girth enhancement procedure performed by a non-medical professional. The lesson is clear: Medical expertise is essential to protecting patient safety.
• Restrict your ability to discuss your experience : NDAs are often requirements of sensitive business negotiations or employment situations to avoid the loss of intellectual property and have no place in a medical office. The sole purpose of an NDA from a girth enhancement is to prevent embarrassing or unflattering results from impacting their business; patients are protected under standard medical privacy protections and contractual agreements with third-parties.
  
  A qualified, ethical girth enhancement provider is willing to stake their professional reputation on their results if needed, and will work to address complications, not put a gag order on their patients.
  

• Promise more filler is always better : Men undergoing girth enhancement will often be eager to get as much done as quickly as possible. While this is understandable, it’s the girth enhancement provider’s responsibility to both set patient expectations and prioritize patient health.
  
  There are many reasons for taking a multi-session approach to girth injections. First and most importantly: There’s a physical limit as to how much hyaluronic acid can be safely injected into a patient before introducing health risks or even death. Second, performing girth enhancement over multiple sessions allows the provider to shape and sculpt the penis and address any issues with its appearance.
  

• Skip your medical history and screening : While properly administered hyaluronic acid-based girth injections carry minimal health risks, patient selection is important for a number of reasons, especially including patient safety. A provider needs to take into consideration factors such as active infections, known allergies, bleeding disorders, and others before injecting into the penis.
  
  This is both a question of patient safety as well as the quality of outcome: In our clinical experience, some patients recovering from surgery or injury appear to lose filler volume sooner. Although this observation has not been confirmed in controlled studies, research shows that oxidative processes can break down HA fillers, providing a possible explanation that warrants further investigation.

Aesthetic clinics will often either fail to ask important questions or not have anyone on staff capable of interpreting the answers to them.

The first time you hold your child, the world can look exactly the same and feel completely different at the same time.

What was familiar, e.g. your job, responsibilities, even the drive home suddenly carry more weight: Your decisions carry another person’s future. You’ll likely think differently about the risks you’re willing to take, or discover a level of patience that you didn’t know you had.

A discussion on the Patrick Bet-David podcast captured that transformation: Becoming a father can shift a man’s attention toward protection, responsibility, and service. The conversation also raised a deeper question. Are those changes simply emotional, or does something biological happen, too?

Research suggests that fatherhood can involve measurable changes in the brain and hormone patterns. For men over 30, that creates an important opportunity to talk about health and how to care for the family without disappearing from your own list of priorities.

“Dad brain” is more than a punch line

We often talk about the biological changes of motherhood. Researchers are discovering that fathers also undergo changes as they enter parenthood.

A study published in Cerebral Cortex followed first-time fathers in Spain and California, scanning their brains before and after their children were born. Researchers found changes in cortical volume, including in networks involved in processing social information and visual input. These were small samples, and the findings do not establish that fathers become smarter, more protective, or better planners. They do support the possibility that becoming a father is a period of adult brain adaptation.[1]

Think of learning to hear your own baby’s cry in a noisy room. Something that once blended into the background becomes personally meaningful. That is a useful analogy for changing attention, and not a claim that researchers have identified a single “dad switch.”

Brain volume reductions also should not be confused with brain damage. Nor should they automatically be celebrated as greater efficiency. A later study linked some changes both to parenting engagement and to sleep and mental health difficulties, but didn’t draw a specific conclusion for good or ill. [2]

Involvement matters: caregiving is something men practice

One of the podcast’s strongest points was that being present matters. Holding, feeding, comforting, playing, and learning a child’s signals are experiences through which a relationship develops.

A 2014 study examined brain responses, oxytocin, and parenting behavior in 89 first-time parents, including fathers serving as primary caregivers. Its findings suggest that paternal caregiving experience is associated with how parenting-related brain networks work together.[3]

This does not mean every diaper change produces a measurable brain improvement. It means men’s capacity for nurturing should not be dismissed as biologically secondary. Caregiving experience appears to matter.

For a father who worries that bonding did not arrive instantly, that is an encouraging idea. A relationship can grow through ordinary repetition: the bedtime routine, the walk after dinner, the quiet moment when you stay instead of reaching for your phone.

Does becoming a father lower testosterone?

It can, on average. That does not happen identically to every man.

In a longitudinal study of 624 young men in the Philippines, men who became partnered fathers experienced larger testosterone declines than men who remained single nonfathers. Fathers reporting more daily childcare also had lower testosterone at follow-up.[4]

Researchers interpret this pattern as consistent with a biological adjustment toward parenting. But it was a population study of young men, using salivary hormone measurements. It cannot predict the testosterone level of an individual 38-year-old father or diagnose a condition requiring treatment.

A change in testosterone after fatherhood is not automatically clinical testosterone deficiency. A hormone number also cannot measure devotion, strength, masculinity, or parenting ability.

The podcast mentions oxytocin and cortisol as well. These systems are part of the scientific discussion about bonding and stress, but their patterns are complex. There is no universal fatherhood hormone profile and no routine “bonding hormone panel” that tells a man whether he is becoming a good dad.

Protection and purpose are real experiences—not universal rules

The podcast uses a racing story to illustrate why a father might reconsider risk. It is a memorable image: when someone depends on you, the consequences of a decision can feel different.

That anecdote is not scientific proof that fathers become more cautious. Some men experience a powerful change in priorities; others describe a gradual shift. Neither response is a test they must pass.

The same caution applies to claims that sons and daughters change men in predictable, separate ways. Children have individual personalities and needs. Gentleness, boundaries, play, and emotional availability matter across genders.

Men without children can also be responsible, nurturing, and deeply committed to others. Fatherhood is one important path through adult development, not the definition of a man’s worth.

Fathers can struggle while loving their families

The transition can bring joy alongside exhaustion, anxiety, isolation, or depression.

A widely cited meta-analysis of 43 studies estimated depression in roughly 10% of fathers during pregnancy and the first postpartum year. Rates varied considerably across studies, so that number is a broad historical estimate rather than a prediction for every family.[5]

Persistent low mood, irritability, withdrawal, or feeling unable to cope deserve a conversation with a healthcare professional. Those experiences should not automatically be blamed on low testosterone—or dismissed as the price of being a provider.

You can love your child completely and still need help.

What this means for men’s health after 30

Fatherhood can be the moment when health becomes about more than appearance. You want the energy to play, the patience to listen, and the years to watch your children grow.

Start with a practical question: What have I been postponing?

It may be a blood pressure check, a conversation about sleep, persistent changes in libido, or help with stress. A comprehensive men’s health assessment can place those concerns in context rather than treating them as isolated problems.

TRT can be appropriate for men with confirmed, symptomatic testosterone deficiency. Becoming a father, by itself, is not an indication for treatment. Clinical guidelines require compatible symptoms and consistently low testosterone, confirmed with repeat morning testing, along with an evaluation of the cause.[6]

You’re attracted to your partner and your libido seems to be as strong as ever. Why is your downstairs not syncing up with what’s going on upstairs?

Difficulty getting or maintaining an erection can be frustrating, but it can also leave you (and your partner) drawing the wrong conclusions: Your partner may think you’ve lost interest, and you may wonder whether or not you need medication or testosterone replacement therapy.

Before either of you settles on an explanation, it helps to understand that wanting sex and getting an erection are different parts of sexual function.

It’s important to note that they’re connected, but one doesn’t guarantee the other.

Desire and Erections: Two Different Questions

Libido is your interest in sex. Erectile function is your ability to get and maintain an erection firm enough for sexual activity.

You can have a strong sex drive and difficulty getting an erection. You can also have reliable erections and very little interest in using them. A medical evaluation considers these separately, along with other concerns such as changes in sensation, orgasm, or ejaculation.

That distinction matters because “my sex life isn’t what it used to be” could describe several different problems. The more clearly you can describe what’s changed, the easier it becomes to investigate why.

Wanting It More Doesn’t Fix Everything

An erection depends on coordinated activity involving your nerves, blood vessels, hormones, and brain. Desire is part of that process, but it can’t simply override everything else happening in and to your body.

Diabetes, high blood pressure, vascular disease, nerve damage, and certain medications can negatively affect erectile function. Stress, anxiety, depression, and excessive alcohol use can also contribute. Sometimes several factors are involved at once.

For a Miami man balancing work, workouts, late dinners, and a busy social calendar, it can be tempting to dismiss a change as the price of keeping up. If erectile problems keep happening, they deserve more attention than another promise to take it easy next weekend.

Do Erectile Difficulties Mean You Need Testosterone?

Not necessarily: Testosterone plays a role in sexual desire and erectile function, and low levels can contribute to sexual symptoms. Erectile issues alone don’t establish that you have low testosterone or that testosterone replacement therapy is the right treatment for you. It’s important to recognize that no two patients are the same, and equally important to have your symptoms evaluated by a qualified medical professional, preferably an andrologist. 

The American Urological Association recommends checking morning total testosterone as part of an erectile dysfunction evaluation. Diagnosing testosterone deficiency requires compatible symptoms or signs and consistently low total testosterone levels, measured on two separate early mornings.

A strong sex drive also doesn’t tell you everything about your hormone levels. The most important thing is to investigate rather than guess.

If low testosterone is part of the problem, treatment may help. If other factors are contributing, those need attention too. A testosterone prescription shouldn’t be expected to resolve every possible cause of erection difficulty.

One Difficult Night Can Become Something You Anticipate

An erection problem can start with an isolated experience and become a source of anxiety the next time you’re intimate with your partner. It’s perfectly understandable how easy it is to get stuck in your own head, with questions like: Is it happening? Is it taking too long? What if I lose it again?

That pressure can interfere with arousal, and anxiety can compound an existing physical problem. Addressing performance anxiety may therefore be part of treatment even when there are also medical factors involved.

If you still have morning erections, or erections are more reliable during masturbation, tell your clinician. Those details are useful clues. They don’t prove that the problem is entirely psychological or rule out a physical contribution.

And if your partner is interpreting erection difficulty as a lack of attraction, a direct conversation may help: “I want to be here. Something isn’t working the way I expect, and I’m going to get it checked.”

What Should You Tell Your Urologist?

Don’t worry about having the proper medical vocabulary to describe what you’re going through. Just be as honest as possible.

Useful details include:

• What changed: Getting an erection, maintaining one, firmness, desire, or more than one of these.
• When it started: Suddenly, gradually, or after a medication change, illness, or procedure.
• When it happens: With a partner, during masturbation, or in every situation.
• What you’re taking: Prescriptions, supplements, hormones, and recreational substances.
• What else you’ve noticed: Pain, curvature, changes in sensation, or other symptoms.

Your clinician may review your medical and sexual history, perform an examination, and order appropriate testing. The goal is to understand the pattern and identify what needs attention, not to pass judgment.

Why It’s Worth Getting Checked

Persistent erection problems can sometimes point to a broader health concern. The American Urological Association identifies erectile dysfunction as a marker of possible underlying cardiovascular disease and other conditions that may warrant evaluation.

While that doesn’t mean every difficult night signals heart disease, it does mean a recurring change is worth discussing rather than quietly accepting it as your new normal.

Treatment depends on what the evaluation finds. It may include addressing underlying conditions, reviewing medications, changing contributing habits, treating performance anxiety, or using medication or other treatments for erectile dysfunction.

Getting back into the dating game in your 40s can be daunting for a number of reasons, especially after a long-term relationship. Dating apps have replaced casual flirting with strangers, said dating apps are awash with catfishing scams, and the general etiquette has shifted in a way that you’re never quite sure if you’re coming across as charming or like a tired old man. 

Those are just the external factors. Next come the ones about your body. What if I can’t perform? Should I worry about STIs? Is my libido lower than it used to be because something’s wrong, or is it just because I’m older? Am I ready for somebody to see me naked again?

Rather than looking at dating again as a source of anxiety, you should instead see it as an adventure, and the perfect opportunity to put more of a focus on your health: After all, the healthier you are, the better you’ll look, feel, and perform.

Start With How You’re Feeling

Rather than focusing on how you look, focus on how you feel.

Are you sleeping well? Do you have energy during the day? Has your interest in sex changed? Have erections become less reliable? Are you taking medications you weren’t taking the last time you were dating?

These are useful questions to bring to a doctor’s appointment, especially if something has changed or is worrisome. Sexual difficulties can be caused by physical health, medications, stress, anxiety, or a combination. Turning 40 doesn’t tell you which one applies.

A New Partner Can Bring New Nerves

You can be excited about someone and still feel anxious about being intimate.

After years with the same partner, familiarity may have made sex feel easier. With someone new, you’re learning each other’s preferences while possibly worrying about your appearance, your performance, and whether you remember how any of this works.

That’s a considerable amount of mental activity for an occasion that is supposed to be enjoyable.

Anxiety and stress can contribute to erectile difficulties, especially when you’ve hit middle age. If something doesn’t go as planned, try not to turn the next encounter into a test you have to pass. Take your time, communicate, and leave room for intimacy that doesn’t depend on a particular outcome. A partner worth dating will understand. What’s more, it’s likely that they’re going through something similar.

Get Tested

If you’ve been in a long-term relationship, STI testing may not have crossed your mind in years. A new partner, or partners means it’s necessary for everyone involved.

Many sexually transmitted infections have no symptoms, and others don’t manifest in the short-term. Talk with your healthcare provider about which tests make sense based on your sexual history, recent exposures, and the kinds of sex you have. Oral or anal exposure may call for throat or rectal testing that a urine or blood test alone won’t cover.

Have the conversation with your partner before you’re in the middle of things. It can be straightforward:

“Before we have sex, I’d like us to talk about testing and protection. When were you last tested?”

(If getting tested seems like a non sequitur to your partner, and they’re not also just getting out of a long-term relationship, take that as a warning sign.)

Condoms and other forms of protection can help reduce STI risk, but they don’t eliminate it. Depending on your circumstances, a clinician can also discuss relevant vaccines and other prevention options. Age and relationship experience don’t make these conversations unnecessary.

Don’t Base Your Self-Image on Online Ads

Once you start thinking about sexual confidence and performance, you’ll quickly come to realize that there’s a massive industry based on capitalizing your self-doubts. 

Products ranging from over-the-counter pills, gas station supplements, and gadgets to sketchy peptides and hormones are pretty much everywhere you turn, especially once online advertising platforms catch you on a dating site. 

Focus on the science and on your actual symptoms: Low sexual desire and difficulty maintaining an erection are different issues, although they can coincide. Erectile problems can involve blood vessels, nerves, hormones, medications, or emotional factors. A proper evaluation helps determine what, if any, treatment is appropriate.

You’re Allowed to Care About Your Appearance

Miami offers plenty of opportunities to compare yourself with people who appear to spend their entire working week at the gym. This can set a nearly impossible standard to anyone with an otherwise busy schedule.

There’s nothing wrong with wanting to look refreshed or exploring an aesthetic treatment. Give yourself time to decide what you want, ask questions, and understand the likely benefits, limitations, risks, and recovery involved.

A looming date is a poor deadline for a medical decision, and there’s no one procedure that can guarantee chemistry with another person.

Take Some Pressure Off Yourself

Getting back into dating is a reasonable time to catch up on routine care, discuss sexual health, and address changes you’ve been quietly worrying about.

Your Sleep Habits Deserve a Closer Look

Sleep is easy to overlook when you’re looking for a treatment that promises to bring your edge back.

Testosterone replacement therapy and other treatments have their place, but they don’t replace sleep.

“If you’re pushing yourself too hard, you’re writing a check that your body won’t be able to cash, at least in the long term,” says Dr. Suarez-Sarmiento, Jr. of the UroFill Clinic.

Don’t Rule Out Sleep Apnea

Spending seven or eight hours in bed doesn’t guarantee restful sleep. If you snore loudly, gasp for air, or wake up exhausted, sleep apnea could be a factor. Your partner may notice the signs before you do.

Untreated sleep apnea can affect more than your energy and sex life. It is associated with high blood pressure, heart rhythm problems, and other cardiovascular risks, so it deserves a proper evaluation.

A sleep specialist can help determine whether you have sleep apnea and recommend treatment, which may include a CPAP machine. Reducing alcohol intake and managing weight can also help, but lifestyle changes shouldn’t replace an evaluation.

You Can Still Enjoy Yourself

Miami has great restaurants and a nightlife worth leaving the house for. We’re not suggesting you live like a monk, but enjoying the city shouldn’t mean running yourself into the ground. Give sleep the same priority you give your workouts.

Your clothes fit better. The scale is moving. The constant thoughts about food have quieted down.

But so has your excitement about other things.

Dinner with friends, your favorite workout, even intimacy. You can still participate, but the enthusiasm feels missing.

Some people taking GLP-1 peptide medications describe this emotional flatness. The experience is real and deserves attention, even though researchers are still determining whether—and how—the medication contributes.

When appetite gets quieter, does everything else?

GLP-1 stands for glucagon-like peptide-1, a hormone involved in appetite and blood sugar regulation. Peptide medications such as semaglutide and tirzepatide act on these signaling systems to help manage metabolic disease and weight.

Their effects involve the brain as well as the digestive system. That has raised questions about whether changes in appetite and reward processing could sometimes extend beyond food. Research on GLP-1 signaling helps explain why scientists are exploring this connection.

Think of it as turning down the volume. Quieting relentless food cravings can bring tremendous relief. But some patients wonder whether the volume has also dropped on the things that make life enjoyable.

That is a useful analogy, but not a proven explanation. To clarify, GLP-1 peptides do not simply “turn off dopamine.”

“I’m not sad. I just don’t care.”

That sentence captures why these symptoms can be difficult to recognize.

Apathy is reduced motivation. Anhedonia is reduced interest or pleasure. Fatigue is low energy. They can overlap, but they are different experiences.

You might want to see friends but feel exhausted. Or you might have enough energy and simply feel no desire to go.

Depression also needs consideration. It can present as loss of interest without obvious sadness, so feeling emotionally flat should never automatically be dismissed as a peptide side effect.

What does the evidence show?

A small 2026 case series described three patients who developed anhedonia-like symptoms during high-dose tirzepatide treatment and improved after individualized treatment changes. That supports further investigation, but it cannot tell us how common this is or prove that the medication caused it.

Meanwhile, the FDA’s comprehensive review found no increased risk of suicidal thoughts or behavior with GLP-1 medications. That is reassuring, although it does not answer every question about motivation or emotional flatness.

What should you do if your spark feels missing?

Tell your prescriber. Be specific about what changed and whether it followed starting treatment or increasing your dose.

The assessment should also consider food intake, hydration, sleep, other medications, and mental health. Strong appetite suppression can leave someone eating too little, which may affect energy and daily functioning.

Years ago, I was in Japan and decided to check out a sushi restaurant that had been recommended to me. Outside of the restaurant was a sign with a picture of a puffer fish, meaning that they had a chef who was certified to prepare it.

Puffer fish, or fugu, is a genus of fish that, as its name implies, puffs up into what looks like a spiky, angry balloon when threatened. It’s an effective warning sign for predators; their livers, ovaries, skin, and eyes carry a potent neurotoxin called tetrodoxin. 

Tetrodoxin is exponentially more potent than cyanide and paralyzes the muscles of whatever consumes it, meaning the mind stays active while the body shuts down around it, making it an extremely unpleasant way to die.

Despite the risk, or perhaps because of it, fugu is also a delicacy in Asia, especially in Japan and Korea. 

What does this have to do with penile girth enhancement safety?

To be able to serve fugu, chefs in Japan need to undergo an apprenticeship for a minimum of three years, and to undergo an exam in how to serve it safely. Only roughly one third of applicants pass the exam. 

Despite the potential deadliness of the toxin, incidents of tetrodoxin poisoning are rare.

The message here is clear: High risk activity requires rigorous training and examination. Chefs need complete knowledge of the anatomy of the fugu, which areas pose the most risk, how to maintain a sterile environment to avoid toxic contamination, and how to safely prepare them for customers.

This rigorous commitment to safety does not extend to penile girth enhancement as it’s practiced around the world and in the United States. How Long Does Testosterone Cream Take to Affect Women?

The answer varies. Some women notice subtle changes within the first month. For others, improvement takes two to three months. A complete assessment may require as long as six months.

These are approximate ranges based on clinical guidance, published sexual-medicine literature, and feedback commonly reported by appropriately selected patients. They are not guarantees. Appropriately dosed testosterone should not make a woman feel masculinized, overstimulated, unusually aggressive, or dramatically different from herself.

The intended effect is a gradual restoration of sexual interest and responsiveness.

Signs that testosterone exposure may be excessive include:

• Acne
• Oily skin
• Increased facial hair
• Increased body hair
• Scalp hair thinning
• Irritability or unexpected mood changes

• Clitoral enlargement

You finish a hard workout feeling stronger, more vascular and completely “pumped”—everywhere, apparently, except below the belt.

If your penis looks temporarily smaller or more retracted after exercise, there is usually no reason to panic. The phenomenon is sometimes jokingly called “gym penis,” but the physiology behind it is very real. More importantly, it is generally temporary, harmless and unrelated to your true penile size or sexual performance.

Why Does It Happen?

During strenuous exercise, your body prioritizes the tissues doing the most work. Blood flow is redirected toward the heart, lungs, skin and active muscle groups. At the same time, intense lifting, cardio and high-adrenaline workouts activate the sympathetic nervous system—your body’s “fight-or-flight” response.

An erection depends heavily on relaxation, healthy blood flow and parasympathetic activity, called the body’s “rest-and-recover” system. A demanding workout temporarily creates almost the opposite physiologic environment.

Think of it as traffic control. During exercise, your body directs circulation toward the busiest highways. The penis is not damaged or permanently losing blood flow; it is simply not the priority at that moment.

The result may be temporary flaccid retraction or “turtling,” in which the penis pulls closer to the body. Some men barely notice it, while others experience a dramatic change.

How Long Does It Last?

For most men, the penis returns to its typical flaccid appearance within several minutes to about an hour after exercise. Recovery may take longer after an unusually intense workout or when other contributing factors are present, including:

• Dehydration
• Cold temperatures

Can Pre-Workout Supplements Make It Worse?

They can.

Many pre-workout products contain high doses of caffeine or other stimulants that increase sympathetic nervous-system activity. These ingredients may improve alertness and exercise intensity, but they can also contribute to vasoconstriction, anxiety, dehydration and temporary penile retraction.

This does not necessarily mean that a pre-workout supplement is dangerous. However, if you regularly feel jittery, experience palpitations or notice that retraction lasts much longer after taking it, review the ingredients and dosage. More stimulants are not always more effective.

The Psychological “Shrinkage” Can Last Longer

Although gym penis is usually physically harmless, the anxiety it creates may not be.

Men often judge themselves based on a single moment in the mirror—or compare their most retracted appearance with someone else’s least retracted appearance. That is an unfair comparison. Flaccid size has tremendous natural variability and is a poor predictor of erect length, girth or sexual function.

Anxiety can then reinforce the problem. The more worried a man becomes, the more activated his sympathetic nervous system becomes, which may prolong retraction and make erections more difficult. What began as a normal physiologic response can turn into a cycle of monitoring and concern.

When Should You See a Urologist?

Temporary retraction that resolves after cooling down and recovering is generally normal. However, schedule an evaluation if you notice:

• A persistent or progressive change in penile size
• New difficulty obtaining or maintaining erections
• Penile or pelvic pain
• New curvature, indentation or hourglass deformity
• Numbness or reduced sensation
• Changes in skin color
• A cold or poorly perfused appearance that does not resolve
• Urinary symptoms
• Retraction that remains pronounced long after the workout
• A bulge, discomfort or difficulty retracting the penis from the surrounding fat pad

These symptoms may point to a different issue, such as erectile dysfunction, Peyronie’s disease, pelvic-floor dysfunction, medication effects, vascular disease or a buried-penis pattern associated with weight gain or an enlarged pubic fat pad.

How to Reduce Post-Workout Retraction

A few simple measures may make “gym penis” less noticeable:

1. Hydrate before, during and after exercise.
2. Avoid excessive caffeine and stimulant-heavy pre-workouts.
3. Cool down gradually instead of stopping abruptly.
4. Change out of tight, damp compression clothing.
5. Take a warm shower after training.
6. Allow your heart rate and nervous system to settle.
7. Stop repeatedly checking; anxiety can prolong the response.

The Bottom Line

“Gym penis” is a temporary response to exercise, and not evidence that the penis has permanently shrunk and not a sign that your workout is damaging your sexual health.

Your body is simply prioritizing performance, circulation and temperature control. Once the workout ends and the body returns to its resting state, the penis should return to its usual flaccid appearance.

The gym may give your muscles a pump while temporarily putting everything else in power-saving mode. That is usually normal physiology. But if the change persists or comes with pain, numbness, curvature, urinary symptoms or erectile dysfunction, it deserves a proper urologic evaluation—not a locker-room diagnosis.
